/eduCommons/personalize_form not functioning properly

Description

Since removing our Registration Tool, the personalize_form no longer matches the default Plone Registration/preferences data.

Notably, first/last name are not retain upon save. Other options need to be reviewed (Country, State, User Role, etc.)

Revised personalize form. Updated to Plone 2 version and modified accordingly to work w/ eduCommons now that we rely on default RegistrationTool? again.
